Disambiguations are pages that include other pages with similar name. For an example, see this page: Moderator (Disambiguation).
How To Use?
Say you wanted to make a disambiguation page about moderators.
1. First of type in search "Moderator (Disambiguation)", without quotations; 2. Press enter then create page; 3. On top, type {{disambiguation | "TITLE WORD"}}. Replace "TITLE WORD" with something multiple pages have in common, in this case it's the word "moderator"; 4. Put link of the pages with same name. Possibly, you should put description and image.
